Policy Acknowledgement Form
This form is required. Complete this form to notify the Office for Study Abroad of your intention to study abroad. Your signature on this form acknowledges your familiarity with and acceptance of study abroad policies and gives the Office for Study Abroad permission to conduct the academic and judicial reviews required for GW approval to study abroad. Submit the form to the Office for Study Abroad along with the approval form from your program application.
Pre-Advising Appointment Worksheet
Complete this form before your first appointment with a study abroad advisor. Return the form to the OSA in order to schedule an appointment and meet with an advisor in the region(s) you are interested in studying abroad.
Course Approval Form
This form is required. Use this form to seek GW course equivalencies and faculty approval for courses you intend to pursue abroad. This form is required along with the official transcript from your study abroad program to process transfer credit. Submit the form to the Office for Study Abroad when you complete the necessary signatures.
Declining Credit Form
This form allows you to decline credit for study abroad courses you do not wish to transfer to your GW transcript.
Survey for Returning Students
Complete this online survey to provide the Office for Study Abroad important information concerning your experience abroad, your assessment of the program, and how your study abroad experience affected your skills and career choices.
Peer Contact Sign Up Form
Complete this application when you return from study abroad if you'd like to apply to participate in the Peer Contact Network. Participants in this program agree to be available to other GW students who have questions about their study abroad experiences and programs. Participants may also be selected to speak on panels regarding study abroad during orientation programs sponsored by the Office for Study Abroad. Submit this form to the Office for Study Abroad.
